There are two different things - Preparing and Working on it.You can start preparing in 4th semester if you plan to give at the end of 7th semester. If you plan to give it in internship, start the hard work from 1st week of January itself.If you are focused right from the beginning that you want to pursue USMLE, then start working on it from 1st semester itself like get involved in research projects with consultants and bag some publications, always be upfront to organize conferences and participate in many extra-curricular activities. Try learning Spanish. It is like a second language in the USA and you will end up getting calls from southern universities more. Take a course of three months(classes only on weekends). It will really make your CV shining.
